All fees must be taken care of prior to our first meet (deadline Wednesday, March 8th).
The Athletic fee is $100 and the ASB card is $40. There are 2 fees required for participation in Track and Field. Students will not be allowed to turn out until all forms have been finalized. If you have filled out the forms for another sport, you will simply need to add Track and Field and do the consent forms.
There are requirements for both Parent/Guardian and Students to sign the forms. It takes about 10-15 minutes - and I HIGHLY ENCOURAGE you to fill the forms out with your student. So no more tracking down random forms, kids losing physical copies, or “accidentally" turning it in to a teacher with their homework. This is in place of the “old” athletic packets and forms. Fall sports at the high school level must be completed by August 8 and junior high August 15. We are asking that all parents of Nixa athletes, both junior high and high school, use FinalForms and complete their account by the first day of their respective practice. If users require any support during the process, scroll to the page bottom and click "Use Support" or contact the athletic department. Please check with your coach/sponsor if you want to check on the status of your current physical date. For new physicals, we will no longer be collecting hard copies of physicals, since parents will be able to take pictures of them and upload those to FinalForms. Since physicals are good for two years, the athletic department will assist in uploading any current physicals to the system, but we cannot complete this task until each parent creates an account for their student(s). FinalForms also pre-populates information wherever possible, for each of your students, saving you time. FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice. FinalForms allows users to complete and sign athletic participation forms for your students, including uploading new physicals. The Nixa School District is excited to announce that the Nixa Public Schools Athletic Department has partnered with FinalForms, an online forms and data management service.
